The Home Study: The Reasonable-Supply Standard: A Pharmacist Framework for Evaluating Disaster and Emergency Medication Preparation Requests

Disasters rarely create new medical problems; they interrupt treatment of existing conditions and delay care for everyday illness, turning managed conditions into emergencies and leaving predictable acute illness unaddressed. Patients request an early refill before an evacuation, or a standby antibiotic before anticipated clinic closures. Existing professional guidance on disaster medication planning, from ASHP and ASPR TRACIE, is scoped to institutional and health-system emergency operations and does not address the individual pharmacist-patient encounter. Meanwhile, real regulatory and clinical precedent for physician-authorized advance access already exists, in state emergency-refill statutes and in guideline-endorsed back-up prescribing, but pharmacists have no structured way to apply that precedent consistently at the counter. The purpose of this application-based activity is to equip pharmacists with a bounded, defensible framework for evaluating these requests: approving what is clinically reasonable, declining what is not, and documenting either decision clearly.
